All of these people have some great advice, take what you need but my strong recommendation would be to watch pro player perspectives. Even though they aren't playing at your skill level you can still watch and learn their positioning, their item builds and how they adjust to different scenarios in the game. It also helps to rewatch your replays (as hard as that might be) and analyse/compare to other games you've done well in. Some games you just can't help. The average player only has between 50-65% win rate anyway so you can't expect to win all the time

Ping the trees and say stand here and harass. If they zone then you must hold equilibrium. They might be thinking that they are going to deny while you last hit, so let them know politely that you've got it.
One important thing is to adjust your item build depending on the game. For example: if they have PA, Windranger, Riki or Broodmother and you happen to be a carry then get an MKB.
If they have AA then DON'T get an armlet.
If they have Medusa, then get a diff blade if its not terrible on your hero, otherwise a necro 3 also works well for burning mana.

But they didn't get one so i was able to tank everything they threw at me and knock down the Tombstone every fight. Once it was down we won fights easily. If they had a diff blade I would have died before taking down the Tombstone and we would have lost.
Also look at what your teammates have before building anything. Having multiple Meks, AC, Vlads, etc is a waste of your team's money.
or they could've pushed and won the game before you became an issue. Wr and luna can take t3s at like minute 25 before medusa is a factor. I wouldn't automatically build diffusal just cus dusa.
^ True. If they ended fast then Diff wouldn't be needed. But if they fail to end it before Medusa gets a few items (I got Maelstrom, Linkens and MKB before they tried to push rax) then Diff is the only item that really lets you kill Medusa in a reasonable amount of time.

I made a fuck around account to play with my 1K MMR friends just the other day, and you wouldn't believe how much 1kers worship Pudge, the moment you miss ONE hook " noob pudge, no hook. " LMAO and this happened every game, cause i played pudge for fun, and every game they would say " nice hook noob. " 1-2kers rage quit more often, they give up more quickly, they flame more often as well. It actually does get better.
i'm at 3.9-4.0 and people at least respect your courier more often (yes, not every game, if it they did every game, then toxicity in dota wouldn't exist, no shit) when in 2K, i can't even count how little respect everyone had for my courier uses, and how quickly they went to call you a noob for missing a stun. (happens in all brackets, but 2K the most, i swear)
when you miss a hook with pudge in my bracket, most people don't really think much of it, they just gather up, see that he missed, and go "oh, he missed, and go back to what they were doing. " lol

Well I was called noob by a weaver who buys same items every game, for buying a battlefury against nature's prophet. He had 1/3 my tower damage and didn't participate in as many kills, but managed to die as much as I did, and I played very liberally. So 2.5k is full of people who are really shit and cannot realize it.
^A sensible LC player will never get bellow 1/3 hp and that is in the early levels, even without BS in the game. But I guess you can't expect people to be sensible @ NS, so they jungle WK and keep almost dying.
U pick wrong heroes.
My suggest is :
If u want play carry,play snowballer heroes like slark,rikkimaru,clinkz,ursa ...
If u want play support,play with agro supports like venom,jakiro,veng,lion ...
earlier i was 300 mmr because of the peenoise but now i have 2,469 mmr. it was so easy to win... just follow these steps. First, mute all annoying players; they will just flame and blame you and leads your team to lose. Second, learn to play meepo or any heroes that can jungle for 10 mins then go for a clash; typical 2k scrubs didnt hunt heroes or even blocking the creep camps so they dont know anything yet but remember "put atleast 1 ward at the rune spawn to avoid ganks. and when you learn how to use meepo... Ez meepo leads to Ez mmr <---- NOT A JOKE just "tab + w" repeatedly and your'e good to go. Lastly, carry your team. 1 clash can turn tables so be there and cooperate. JUST PLAY LIKE A TEAM AND HAVE TEAMWORK.
Play anything you want, any hero can win 65% in below 4kmmr. Just watch replay after every lost game and after a game that you feel you are really good. When watching replays, watch your last hits in laning phase to memory your mistakes, find out and remember how to LH under all the hard-to-last hit situations (under tower, with multiple ranged creeps, with catapults,etc). Watch every death of you 2 times, first time in your own player perspective, second in your killer perspective. Think how can you avoid that death: wards, calm, concentrate, use spells effectively,... Most of you death can be learned and improved. Some of them are inevitable.
TL,DR: When you watch replays, you can easily spot your mistakes in details, so you can fix it afterward.

The way to get out of 2k is pretend your teammates are bots on easy mode and you have to carry your ass off.
Don't die EVER.
Get tons of last hits.
If your teammates are feeding be positive with them n tell them to farm jungle.
If enemies are ganking heavy, or you fear ganks, buy wards yourself. Noob 2k supports will say they need money for "X" item so they can't buy wards.
Doesn't matter if you escape 2k cause when you get to 3k games are unwinnable, every match all your teammates are full of people with 350 GPM average on carries who smurfed into 3.5k and are on their way down.
Dude you played only 250 games you should just focus on having fun and if you want to get better at the game (if you get better at the game you'll get more mmr) by looking at pro games and downloading them and looking at POV (point of view of the pro player).And the most important thing you CANT do in a game if you want to win is tilting, its better if you're drunk.

Getting out of 2k is basically finding your best hero and used him or her to go to 3k. I used Invoker offlane for climbing back to 3k after a series of losing streaks due to a bad day.
I'm a 2k plebe myself and I have the same issue, if you play offlane you can zone the enemy carry and make space for your other team mates. ask you team if you can be solo bot so you can also have effect late game. if someone else pickes carry offlane go support the carry. Always pick a space creating hero. Also try not to flame and keep on making sure everyone communicates and that last thing is the hardest part because in low mmr games everyone doesn't think about other options and ignores you. And again im also low mmr so im not an expert of getting high mmr :D! this is my way

get good at one hero and spam it. imo dragon knight is a really good choice this patch. tanky and being able to push down towers.

You can't win 1v5 with Storm, you need to play Spectre
i like playing spectra but my main isue with that hero is that it requires compitent suports which are a rare comodity
