As i know, noone expect valve knows exactly but the things you mentioned have something to do with it. First of all games are stil based on rating not ranks. On the top right corner in your stat profile ingame you can see your rating. Your rating will result in a rank.

here you can see which mmr is which rank.
BUT there is an additional possibility to get an higher rank. Even if you are in 2k mmr you can get Ancient5 for example if you play like a very high skilled player ( high kda gpm/xpm, maybe warding dewarding, stacking and denying) for lets say the next 10-20 games. And you can lose mmr but you cant lose your rank anymore. Thats how it works as i know it.

The only thing that Valve calculates is +- 25 MMR. You can feed 0-50 every game but if you win your MMR goes up. There is no "play good and rank up, place wards and rank up, get good Dmg and KDA and rank up"
Literally if you win you go up you lose you go down. There is no fucking mystery or algorithm

But your icon your "rank" can go much higher then your mmr is. I got Archon 3 last season while i was way more down in mmr (like 2,1k while you need to get something like 2,9k mmr to reach it without playing over average.
So yes, you win, you gain mmr, you lose, you lose mmr, always about 25. But your icon, your rank can be higher then that.
Whenever I'm playing above average and win a hard game I tend to rank up, at least it was that way last season, so I think lorn anvil may be right
